1.Once the form has been filled online, take a printout of the form, affix the photograph and put your signatures at the designated places.
2.Send the duly filled in form to the Coordinator UPPGMET-2012, First Floor, Anand Ashram Complex, Rampur Garden, Bareilly - 243001 through speed post so as to reach latest by February 10, 2012 (Friday)
3. Attach the following with the application form:
A demand draft of
Rs. 1000/-
in favour of "UPUMCWA" payable at Bareilly. Write your name and application form number at the back of the demand draft
- One Self addressed Envelope : 10"x4"
- Four colour photographs (2.5" × 2")
- Photocopies of the marksheets of 10th, 12th, and M.B.B.S. Final & Pre-Final Prof.
